This Privacy Policy explains how Couvaro (“Couvaro,” “we,” “us,” or “our”) collects, uses, and shares information when you use our websites, forms, and related services (the “Services”), including coverage and lead submission forms operated on behalf of our partners.

Information you provide

- Profile and eligibility details such as biological gender at birth, tobacco or nicotine use, visit or membership type, and similar information requested by a form.
- Signature and acknowledgment records such as an electronic signature, waiver acceptance, and the consents you provide.
Information collected automatically
- Device and usage data such as IP address, browser type, device identifiers, pages viewed, and interactions with a form.
- Consent and session records. We use third-party tools, including ActiveProspect TrustedForm, to create an independent, tamper-evident record of your form session and the consent you provide. This may include a recording of the page, your interactions, the consent language displayed, timestamps, and your IP address. See “Third-Party Services” below.
- Cookies and similar technologies. We and our providers may use cookies and similar technologies to operate and secure the Services and to measure performance.
2. How We Use Information
- To provide, operate, and improve the Services and to process your submission.
- To activate, document, and administer coverage, waivers, and acknowledgments.
- To contact you, and to enable our partners and licensed insurance producers to contact you, about insurance products and services, including by phone, email, and text message, where you have provided consent.
- To create and maintain consent and compliance records.
- To detect, prevent, and address fraud, security, and technical issues.
- To comply with legal obligations and enforce our terms.
3. How We Share Information
We may share your information with:
- Insurance carriers, agencies, producers, and marketing partners. When you submit a form and provide the applicable consent, we may share your contact and submission information with one or more insurance carriers, licensed insurance producers, agencies, and their authorized partners so that they may contact you about, and provide, products and services. These parties may include the entities identified in the consent disclosure shown to you at the time of submission.
- Service providers. Vendors who perform services for us, such as hosting, email delivery, document generation, lead distribution, and consent verification (for example, ActiveProspect TrustedForm).
- The venue or program operator associated with the form you completed (for example, a gym or wellness facility), for recordkeeping and coverage administration.
- Legal and safety. When required by law, legal process, or to protect rights, safety, and property.
- Business transfers. In connection with a merger, acquisition, financing, or sale of assets.
4. Communications and Your Consent
If you provide consent on a form, you agree that we and the partners identified in the disclosure may contact you at the phone number and email address you provide, including by automated telephone dialing systems, prerecorded or artificial voice messages, artificial intelligence, email, and text message, for marketing purposes. Consent is not a condition of purchase. Message and data rates may apply, and message frequency may vary. You may opt out of text messages at any time by replying STOP, and reply HELP for help. You may opt out of marketing emails by using the unsubscribe link in those emails. Opting out of marketing does not affect transactional or service messages related to a submission you made.
5. Third-Party Services
We use ActiveProspect TrustedForm to independently document your consent and form session. Your use of a form is also subject to ActiveProspect’s privacy practices. The Services may link to third-party sites and partners that operate under their own privacy policies; we are not responsible for those practices.
6. Data Retention
We retain information for as long as needed to provide the Services, document consent, comply with legal and contractual obligations, resolve disputes, and enforce our agreements, after which we delete or de-identify it.
7. Security
We use reasonable administrative, technical, and physical safeguards designed to protect information. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
8. Your Choices and Rights
You may request access to, correction of, or deletion of your personal information, and you may opt out of marketing communications as described above. 9. Children’s Privacy
